Prices marked as starting at may increase depending on visit complexity, testing, procedures, or outside laboratory and interpretation fees.
Paperwork & Forms: Paperwork fees apply to self-pay and commercially insured patients. Medicaid patients are exempt. Some paperwork may be included at no additional charge when presented during a scheduled office visit.
After-Hours Nurse Triage: Calls to our after-hours line that require nurse triage will incur an additional fee. This fee applies to both self-pay and commercially insured patients. Medicaid patients are exempt from this charge.
After-hours nurse triage fees are separate from regular office visit charges and are due regardless of whether an in-office follow-up is recommended.
